Beyond Ambient: Navigating Ethically Built AI Implementation in Behavioral Health

Most behavioral health organizations aren’t failing at AI because they chose the wrong solution. They’re failing because they bought the solution before they built the governance. Beyond Ambient is a white paper from Core Solutions, developed in partnership with OPEN MINDS and the Core Solutions AI Advisory Board, that gives behavioral health leaders a practical, field-tested framework for implementing AI in a way that earns clinician trust, protects individuals, and delivers lasting value — not a six-month win followed by a fragmented portfolio of solutions nobody uses.

This white paper is essential reading if you want to understand:

  • Why AI governance in behavioral health is different from general healthcare — including the primacy of the therapeutic alliance, the unique sensitivity of mental health and SUD records, and why clinical judgment resists standardization in ways AI cannot replicate
  • How to structure an AI governance committee with real authority — who belongs at the table, what decision-making power they need, and why the frontline clinician seat is the one most often cut and the one that matters most
  • A four-gate lifecycle framework every AI solution must pass through — evaluate, approve, monitor, and sunset — including the gate most organizations skip that quietly accumulates risk
  • The specific compliance risks ambient AI creates under 42 CFR Part 2 — now enforceable as of February 16, 2026 — and what to look for in legal counsel who can evaluate vendor contracts with an AI lens
  • How to measure what actually matters beyond minutes saved — alliance quality, override and correction rates, treatment plan fidelity, and disparity analysis across the populations you serve

The question most behavioral health C-suites faced not long ago was whether to adopt AI at all. That question is settled. A 2025 APA Practitioner Pulse Survey found that 56% of psychologists used an AI solution in their practice during the year, nearly double the rate from the year before. A 2026 KFF tracking poll found that 1 in 6 U.S. adults had already turned to AI for mental health information or advice — rising to 3 in 10 among adults under 30, and highest among those with the least access to traditional care. The individuals you serve are already turning to AI for mental health information — outside your organization, often with the least safety net when something goes wrong.

How to do this without making things worse remains the open question. This white paper addresses that question with the specificity behavioral health requires. Drawing on the Joint Commission and Coalition for Health AI’s Responsible Use of AI in Healthcare guidance, the NIST AI Risk Management Framework, and the lived experience of behavioral health leaders on the Core Solutions AI Advisory Board, it presents a framework any organization can follow, organized around asking the right questions in the right sequence rather than prescribing a single model.

The organizations getting real value from AI are not exceptional in resources or sophistication. They are exceptional in sequence. They built governance before they bought solutions.
